I charge around $20-25 per page for transcribing a lead sheet up to
publishing standards utilizing Finale from a recording. I believe that price
is fairly typical. I agree with Rachelle that it is sometimes worth it to
have someone who is knowledgeable do your lead sheets for you. (Note that I
am NOT soliciting business here. My plate's full enough at the moment, thank
you.)
As Rachelle said, there are probably people out there who do this kind of
work more as an avocation and would likely be less expensive. I think that
would be fine for creating generic lead sheets. But if you plan to publish,
go to someone who is up to those standards.

In response to Judy's message:
It took me a long time to get to the point where I could efficiently notate
my own lead sheets and write choral arrangements. I would suggest to
people who have loads of experience as a song leader but are not highly
trained in theory to ask someone who is to write lead sheets for them.
Payment or barter doesn't have to be exorbitant, especially for those who
study music or pursue it as an avocation.
I do think teachers welcome good materials. One of the things I do is give
seminars to pre-school teachers who want to improve their musical skills.
What I find, though, is that while written music is good, most need a tape
to go along with it, since most teachers are not musically trained. I
would love to see that teachers' guide, Judy.
