form 2688 requires a very good reason, must be accompanied by affidavit's 
signed by David and your rabbi that you have attended services every day for 
all three services, and therefore have  been too busy to file form 4868.  Of 
course if you attended all three services every day, you most likely don't need 
the extension....(catch 22)

Monday (Hoshana Rabba) is the official cut-off date.
Hard-core Lubavitchers maintain that the grace period extends through
Channukah.
Filing a form GOD-4868 gets you an extension to Purim. (On the rabbinic
analogy of Yom Kippur to "Yom ki-Purim".)
Filing a form GOD-2688 =may= get you a longer extension, but first check
with your accountant.
