The disc is Kumi Ori and her name is Naomi Teplow.  Musical Production by
Moshe Wohl.  I don't see an address on the disc, but alas, cannot read all the
Hebrew.  It may be there.

I purchased mine at Frank's Hebrew Bookstore in Cleveland, OH.  They do have a

On a personal note - this is a beautiful, moving c/d, my new favorite!
