This discussion started me thinking about the differences! in NZ, it is the
custom to give a (not The) mishabeirach for every aliyah to the Torah. I
gather from this discussion that the term is often applied in the US to the
blessing for a refuah shlemah. That being sorted, our custom is to use a
list, but also to ask for names to be included as we come to the prayer. In
a small setting, which we are, this works well.
With Yahrzeit lists, we use a list, and generally mention the relationship,
as again our list is not huge. We also ask for names in case there are
visitors, or names which have been missed.

We do not have a permanent Mi Shebeirach list. Every week, we have
a Mi Shebeirach sheet that service attendees may sign. We also include
the names of people whom our members tell us they would like on the
list, be it friends or relatives, etc. We do have a hospital list, but,
some people don't want their names on any list and are uncomfortable
having them read in public. That way, our list is current and relevant
to our congregants' needs. If there are visitors, we sometimes will ask
for names from the congregation, but, usually, these people have already
put names on our weekly list as they have come into the sanctuary.
I can send the list an attachment of the Sheet we use if you like.
