In our synagogue we don't have a 'permanent' Mi Shebeirach list.  There's a 
pause just before the prayer & song with an invitation to offer names, but no 
permanent list.
 
As for relationships during the Kaddish names, the Rabbi states them for the 
recent deaths, but not the Yertzeits.
