Everyone I've spoken to has had the same problem - including myself.
Transcontinental make a huge mistake in this type of binding, and my pages
started coming out immediately.  Unless you want to pay shipping and loss of
time to send it back - and hope you get a better-bound copy (which I doubt
you will) here's what I did:  I cut the binding, put the music in page
protectors, and got a large 3-ring binder. You can slide the cardboard title
page into the front cover of your binder.
