Hevra,
I am researching Jewish sources and gathering information on name change and 
addition.  I would like to know about minhagim (customs) of different 
communities, historical and contemporary practices and ceremonies and from 
where these derive precedent.  I know that many people in our community 
choose new names at critical life junctures--whether they are embarking on a 
spiritual journey or facing illness in addition to the more common namings 
and changes around birth and marriage.  I am interested in scholarly 
information as well as personal anecdotes.  Any help you can provide would be
