[HANASHIR:4412] Who wrote these?

I try to give correct attribution to songwriters in the temple's service 
folder.  But I get different answers depending on where I look!  I'd 
appreciate hearing from anyone who knows who really wrote the following:

1-Maoz Tzur -- G.Gottheil & M. Jastrow?  Or traditional?

2-O Chanukah (Y'mey Hachanukah) -- A.Evronin & E.Guthmann? Or tradit?

3-Maccabee March (on Cindy Paley's "Celebrate With Cindy, Vol. 1")

4-The kids' song, "Who gave us Chanukah, tell me please.  I know, I know, 
the Maccabees."  And its correct title.

5-The Shabbat candle blessing (not from Gates of Song)that goes,
  "Come let us welcome the Sabbath, shalom, Shabbat, shalom.
   Baruch atah...
   Now as we kindle the candles, shalom, Shabbat, shalom.
   Baruch atah..."
   BTW, has anyone ever seen sheet music for this?
