Abby,

The Tot Shabbat service that I lead was modeled originially from a small
booklet called Shabbat: A Family Service, by Judith Abrams, illustrated by
Katherine Janus Kahn, and published by Kar-Ben Copies, Inc., Rockville, MD.
I'm sure you can order it from Israel Bookshop in Boston.

I don't follow it to the letter....I used it as a framework from which to
build my service. 

I don't use any siddurs during our service, as I feel it would be a
distraction. My service is very interactive and includes LOTS of movement, and
getting up and down, and so a booklet would just get in the way. However, I've
seen Carol Boyd Leon's siddur that she created, and it is great and an
excellent model to use if you want to use a booklet/siddur.
