I posted an answer last week, but it looks like it didn't come through - so...

The proper credit for Mah Tovu's Hashkeeveynu/Sh'ma is:

(Note that the proper title/spelling is "Hashkeeveynu/Sh'ma").

And, by the way, a lead sheet of Hashkeeveynu/Sh'ma is available for anyone 
who is interested. There is no cost, but we do ask that you make a $5.00
