To my knowledege, there is no sheet music yet for "Modim" -- but there is a 
songbook rumored to be in the works upon his return (transcribed and notated by 
Eric Komar).  There is sheet music for "Halleli" in the Ruach 5761/5763 
songbook 
(http://www.uahc.org/cgi-bin/hazel/hazel.cgi?theme=trans&action=DETAIL&ITEM=993221),
 which includes both Ruach CDs -- such a bargain!

As for lead sheets, give me a buzz on my cell or at home and I can get them to 
you mid-week (Tuesday or Wednesday).  If you don't already have the Ruach 
songbook, you can borrow that as well (until you purchase your own copy of 
course :-)! ).
