[HANASHIR:8820] tattered kaddish

shalom y'all,
does anyone have a copy of the reading entitled (i think) "a tattered 
kaddish" - 
it intersperses the names of concentration camps with the words of kaddish 
yatom.
i'd like to use it for our yom hashoah service.
