[HANASHIR:6731] Lyrics attribution help

I'm preparing to reprint my "Songs From The Heart: Family Shabbat" songbook 
for distribution by Tara and would like to give proper attribution to the 
author of the lyrics to one of my songs, "When God Made The World."  The 
words appear in Gates of Prayer but I remember being told by someone on this 
list that I should put the author's name on that sheet music.  Can someone 
tell me who wrote the words?  Should attribution be given for words like 
those that are part of the liturgy?
