This is Psalm 150.  The text may be found in, as far as I know, any siddur,
in the Psukey d'zimra.  I don't know whose version this is, but it is a
fairly common version.

Ki Va Moed may be found in the Shlomo Carlebach Anthology, obtainable from
Tara.
